We are seeking a Care Manager for our lovely facility at Kensington

 

About the role:

This role is supports the facility General Manager with clinical and management responsibilities to ensure delivery of quality clinical and person centred care for our residents. The role requires some rotating rostered weekend work.

About you:

Essential Criteria:

  • Bachelor Degree in Nursing
  • Current Registration with AHPRA.
  • Minimum 5 years’ experience as a RN in aged care in an Australian setting and 3 years’ experience in management in either residential or acute care hospital.
  • Strong Gerontology competence
  • High level computer literacy
  • Passionate regarding resident lifestyle and well being
  • Strong skills in clinical assessments and care coordination
  • Understanding of Accreditation requirements and quality improvement
  • Demonstrate strong leadership and change management
  • Demonstrated ability to coordinate services
  • Strong skills to manage multidisciplinary team
  • Experience in Roster Management
  • Have an excellent understanding of Aged Care Funding Instrument (ACFI), continuous improvement, WH&S and risk management

  Desirable Criteria:

  • Ability to work in a culturally diverse environment
  • Understanding and empathy with religious based service delivery
  • Proficiency in spoken Greek and Greek cultural appreciation is an advantage.
